What Is Leaky Gut?

"Leaky gut" is a popular term used to describe increased intestinal permeability — a condition in which the protective lining of the digestive tract becomes less selective about what passes from the intestines into the bloodstream.

Under normal circumstances, the intestinal lining serves as a highly sophisticated barrier. It allows nutrients, vitamins, minerals, amino acids, and other beneficial compounds to be absorbed while helping to keep unwanted substances confined within the digestive tract.

This protective barrier is maintained by structures known as tight junctions. These microscopic connections between intestinal cells help regulate what enters the body and what stays within the gut.

When these tight junctions become compromised, substances that would normally remain inside the digestive tract may pass through the intestinal barrier and enter circulation. This phenomenon is commonly referred to as "leaky gut."

Once these substances enter the bloodstream, the immune system may respond with inflammation. Researchers continue to investigate whether intestinal permeability contributes to chronic inflammatory and autoimmune conditions including celiac disease, Crohn's disease, inflammatory bowel disease, and other immune-related disorders.

Researchers have identified several mechanisms that regulate the intestinal barrier, including a protein called zonulin, which influences the opening and closing of tight junctions. Scientists now recognize that disturbances of the intestinal barrier can occur in a variety of digestive and autoimmune conditions.

Research published in Frontiers in Immunology suggests that disruption of the intestinal barrier may play an important role in the development and progression of autoimmune diseases in genetically susceptible individuals. The authors note that a compromised intestinal barrier may allow toxins, antigens, and bacteria to enter the bloodstream and contribute to immune dysregulation.

Researchers have also identified connections between gut barrier dysfunction, the gut microbiome, immune regulation, and chronic inflammation. According to Dr. Alessio Fasano, one of the leading researchers in the field of intestinal permeability, the intestinal barrier plays a central role in regulating what enters the body from the digestive tract.

Although scientists continue to debate whether leaky gut is a cause of disease, a consequence of disease, or both, maintaining a healthy intestinal barrier is increasingly recognized as an important component of overall wellness.

Many factors can affect intestinal permeability. In many cases, it is not a single issue but rather the cumulative effect of multiple stressors acting on the digestive system over time. Common contributors include:

  • diets high in processed foods, refined sugar, and alcohol
  • chronic stress
  • imbalances in the gut microbiome
  • certain medications, including antibiotics and long-term use of NSAIDs
  • nutrient deficiencies, particularly zinc, vitamin A, and vitamin D
  • digestive infections and inflammatory conditions
  • poor sleep and unhealthy lifestyle habits

Over time, these factors may place stress on the digestive system and affect the integrity of the gut lining. Supporting a healthy diet, balanced microbiome, stress management, and overall wellness can help maintain normal gut barrier function.

There is currently no single symptom that confirms leaky gut. However, many individuals experiencing intestinal permeability report a collection of digestive, immune, and inflammatory symptoms.

Symptoms that are commonly associated with leaky gut include bloating, gas, food sensitivities, digestive discomfort, diarrhea, and/or constipation.

Some people also experience symptoms beyond the digestive tract, including fatigue, brain fog, autoimmune conditions, and general feelings of poor well-being. These symptoms do not necessarily mean someone has increased intestinal permeability. However, they often indicate that digestive health deserves closer attention. Over the past 25 years, I have found that one of the most powerful ways to support digestive healing is to reduce the digestive burden on the body while increasing nutrient density. This philosophy forms the foundation of my book, Ten Days to Optimal Health.

Rather than focusing on a single "leaky gut diet," I encourage clients to adopt a short-term therapeutic eating plan that removes common inflammatory foods while providing the nutrients needed to support the intestinal lining and healthy digestion.
